RETURN MERCHANDISE FORM

PLEASE READ OUR RETURN POLICY:  To return an item please print out this form and fill in the information.  Please note: All sales are final on personalized items. No returns, exchanges, or credits.  Mail this form with the item(s) to be returned to the address above.  The item(s) must be returned within 30 days, and a credit will be issued.  If you are returning an item for a refund and originally paid by credit card, we will issue a credit on that same credit card account.  If you originally faxed your order and paid by check or money order, we will send a refund check.  To exchange one product for another, please fill out the exchange form below.  All returns must be accompanied by this form.  All returns and other claims must be made within 30 days of the invoice date.  No returns or claims can be made after that time.  Items sold in sets, must be returned in their entirety.  When returning damaged items, please indicate if you would like a refund or a replacement item.  When figuring your refund or exchange total, please remember any discounts or OH tax that applied to your original order.  Returned items must be in resaleable packages, unless defective.  Original freight charges are non-refundable,  Please add freight charges to exchanges or additional orders.  For your own protection we suggest that you return VIA UPS OR INSURED PARCEL POST.
